    Nasa has labelled the botched 2024 Starliner mission, which left two astronauts stranded in space for months, a "Type A" mishap, on par with fatal shuttle disasters of the past, in a newly published report.

    The category is the space agency's most severe, reserved for incidents causing more than $2m (£1.49 m) in damage, the loss of a vehicle or its control, or deaths.

    On Thursday, Nasa's new boss, Jared Isaacman, blasted Boeing, which built Starliner, and the space agency for poor decision-making and leadership that led to the failed mission.

    The incident drew global attention after the pilots were stuck for more than nine months in space before returning home last March.

That’s the major takeaway from a report released on Thursday by the space agency that summarizes investigations—some still ongoing—of what went wrong before, during and after the botched crewed mission to test the readiness of Boeing’s Starliner spacecraft to ferry astronauts to and from the ISS.

    NASA Releases Report on Starliner Crewed Flight Test Investigation

    At a news conference on Thursday, NASA released a report of findings from the Program Investigation Team examining the Boeing CST-100 Starliner Crewed Flight Test as part of the agency’s Commercial Crew Program.  

    “The Boeing Starliner spacecraft has faced challenges throughout its uncrewed and most recent crewed missions. While Boeing built Starliner, NASA accepted it and launched two astronauts to space. The technical difficulties encountered during docking with the International Space Station were very apparent,” said NASA Administrator Jared Isaacman.  

    “To undertake missions that change the world, we must be transparent about both our successes and our shortcomings. We have to own our mistakes and ensure they never happen again. Beyond technical issues, it is clear that NASA permitted overarching programmatic objectives of having two providers capable of transporting astronauts to-and-from orbit, influence engineering and operational decisions, especially during and immediately after the mission. We are correcting those mistakes. Today, we are formally declaring a Type A mishap and ensuring leadership accountability so situations like this never reoccur. We look forward to working with Boeing as both organizations implement corrective actions and return Starliner to flight only when ready.”

    Grammys executive producer Ben Winston opened up about Bieber’s performance at the awards show during a Monday, February 2, episode of Rolling Stone’s “Music Now” podcast. “I don’t think he decided what the outfit would be until he walked on stage,” Winston said of Bieber’s look, which included nothing but his boxers and socks.

    When Bieber, 31, rehearsed for the show, Winston explained that the singer wasn’t saying, “This is what I’m going to do.”

    As it stands, Amazon said Season 2 of Fallout is the top-performing season on Prime Video since Reacher Season 3, which debuted in February 2025. The streamer added that it is also currently the No. 6 TV season of all time on Prime Video. It falls behind both Seasons 1 and 2 of Lord of the Rings: The Rings of Power, Reacher Seasons 2 and 3, and the first season of Fallout.

    Fallout Season 2 is Prime Video's sixth most-watched series of all time

    Over half of viewers are outside the US, biggest audiences residing in UK, Germany, and Brazil

    Fallout season 2 has already become Prime Video's sixth most-watched series of all time in four weeks.

    As reported by Deadline, Fallout's second season ranks behind both series of Lord of the Rings: The Rings of Power, Reacher seasons two and three, and the first season of Fallout.

    Looking at just the returning seasons chart, Fallout ranks at No.4 behind Reacher and Rings of Power.

    Fallout season 2 premiered on December 16, 2025, with episodes airing weekly as opposed to its first season in 2024.

    Amazon has yet to release the full viewing figures for Fallout season 2, but has shared some interesting stats including that a sizable chunk of its audience reside in the UK, Germany, and Brazil.

    Streaming Ratings: ‘Fallout’ Falls Off With Change From Binge to Weekly Release for Season 2

    The Nielsen streaming stats for the season two premiere of Fallout are significantly smaller than those for season one in 2024 — but that’s hardly the complete picture.

    Fallout accumulated 794 million viewing minutes on Prime Video for the week of Dec. 15-21; season two debuted on Dec. 16. In raw numbers, that’s way down from the 2.9 billion minutes of watch time for season one’s opening week in April 2024, which stands as the best premiere for a series ever on the Amazon-owned platform.

    There are a couple of big differences between the seasons, however, that help account for the disparity. Prime Video released Fallout season one as a binge. In season two, the streamer opted for a weekly release, and it dropped only one episode on Dec. 16, rather than two or three as it has done for a number of other series.

    Fox News chief foreign correspondent Trey Yingst reports on growing unrest in Iran as anti-government protests enter their third week on ‘Fox Report.’ 

    President Donald Trump said Sunday that Iran is "starting to" cross U.S. red lines, citing reports of civilian deaths and warning that any attack on American interests would be met with overwhelming force as his administration weighs what he called "very strong options."

    Trump is pledging to ‘rescue’ protesters in Iran. He has a selective view of the sanctity of protests.

    President Donald Trump billed strikes on Venezuela earlier this month as being about drugs, gangs invading the United States and law enforcement – i.e. arresting indicted leader Nicolás Maduro. But of late, he seems conspicuously preoccupied with the country’s oil wealth.

    And the American people seem to have noticed. A CBS News poll last week showed many Americans thought the administration’s goals in Venezuela were about oil (59% saying the goals were “a lot” about access to oil) or about expanding US power (51%) – more so than drugs (38%), an invasion of gangs and terrorists (37%) or enforcing the law (31%).

    That’s pretty stunning. It took years for the idea to take hold that George W. Bush’s administration invaded Iraq under false pretenses (about weapons of mass destruction); Americans are already largely there when it comes to Venezuela.
